We analyzed 765,762 Shopify stores with app records on June 30, 2026. The tracked form-builder cluster included Powerful Contact Form, Hulk Form Builder, HulkApps Form Builder, Globo Form Builder, Jotform, Nerdy Form Builder, Contact Form Ultimate, Upform, POWR Form Builder, Powr Form Builder, and several long-tail form-builder names.

We cannot fully detect Shopify Forms installs that leave no public app-name signal, theme-native Liquid forms, custom private forms, unrecognized Typeform or CRM embeds, or whether a form is active and converting.
So read this as a study of StoreInspect-visible form builder app adoption, not a complete census of every Shopify store that has any contact form.

Powerful Contact Form is the largest visible app-name signal by a wide margin, and the current Shopify App Store path we verified is Powerful Contact Form Builder. That makes it both a current buyer shortlist and a useful audit signal for stores that already expose this app name.
Hulk Form Builder is the next current App Store candidate in this dataset. The separate Globo Form Builder row is still useful, but we treat it as a StoreInspect app-name signal rather than a separately verified current App Store recommendation.
Jotform and POWR are different. They often behave like embedded widgets or external form systems. That can be useful when the business already runs operations in those tools, but it can also mean the form workflow is less native to Shopify's customer, order, and marketing data.
Shopify Forms deserves its own caveat because it is both important and hard to measure with the same method.
For a simple form, Shopify's native app is usually the first thing to test. It is close to the platform, it can work well for lead capture, and it avoids adding another large third-party script just to collect an email address or basic customer details.
Native covers the baseline, but richer form workflows still need more.

The adoption curve is the main story. Only 1.087% of under-50K stores show a detectable form builder, but adoption reaches 7.046% in the 200K to 1M tier.
That fits the economics. A tiny store can handle weird requests manually. A scaling store needs better routing, cleaner qualification, and less inbox chaos. Once traffic increases, every vague "contact us" form creates operational drag. The store needs to know whether the shopper is asking for support, wholesale access, custom manufacturing, a bulk quote, a partnership, a return, or an order issue.

Stores with 1,000+ products are about 2.1x more likely to show a form builder than stores with under 25 products. That makes sense. More products usually means more edge cases: compatibility questions, replacement parts, custom configurations, B2B requests, stockist questions, warranty claims, and quote requests.
But product count alone is not enough. A 40-product industrial parts store can have a bigger form need than a 2,000-SKU apparel catalog with straightforward purchasing. The better filter is category plus product count plus traffic plus existing app stack.

Fashion has the most form-builder stores by raw count, but Hardware & Tools, Office & Packaging, Events & Tickets, Services, Automotive, and Electronics show higher adoption.

Form-builder stores are more likely to have paid or custom themes, 50K+ traffic, Shopify Plus signals, larger catalogs, and deeper adjacent app stacks.
That makes sense. A merchant does not usually add a form builder because the store is simple. They add it because the workflow has become specific: wholesale request, file upload, custom product intake, consultation request, support routing, product inquiry, or B2B approval.

The email and reviews pairings are especially important. A form is rarely just a page element. It is a data handoff. If the store already runs Klaviyo, Omnisend, Judge.me, Yotpo Reviews, Loox, PageFly, Shogun, or Gorgias, the form should probably feed a better workflow than a generic inbox.

Hulk Form Builder is the strongest current shortlist from the visible adoption data. We found 4,352 stores using the Hulk Form Builder group, including 3,266 with 50K+ traffic and 3,588 with at least one contact.
It fits stores that need more than a contact box: quote forms, custom fields, surveys, wholesale applications, support routing, product inquiry forms, and file-upload workflows. If the form sits on product pages, landing pages, or B2B pages, test theme compatibility and page speed before rolling it across the whole store.
We found 3,505 stores with the Globo Form Builder app name, including 2,451 with 50K+ traffic.
Use that row for audits, migration research, and competitive analysis. Before recommending it as a separate current app choice, verify the merchant-facing vendor path and support status.
Jotform appears on 239 stores in this dataset. It can make sense when the business already uses Jotform for applications, surveys, event registration, service intake, or internal routing. If submissions need to become Shopify customer tags, email segments, quote requests, support tickets, or order-context records, verify the handoff before committing.
POWR is a multi-widget platform, and POWR Form Builder appears as a small specific form-builder signal in our current app-name data. It can fit stores already using POWR widgets or lightweight embedded forms, but test page speed, mobile layout, styling, and data routing.
For agencies, app founders, and B2B teams, the useful question is not "which form app is biggest?" It is "which stores have enough complexity to need better intake, but no detected form builder?"
